Primary Care Fees
(Additional $30.00 for every 5 minutes over 60 minutes)
You must call AT LEAST 24 HOURS BEFORE YOUR APPOINTMENT TO CANCEL without being charged a fee.
Repeated cancellations may lead to your being dismissed from Primary Care.
Missed Primary Care appointments for any reason without prior notice (24 business hours) will be charged a missed appointment fee
